This form is for the student who wishes to re-take the courses of next semester (12-13Autumn-Winter Semester) . Student need to fill in the form and return it to your coordinator by June 18, 2012. Student needs to get a written consent from corresponding department before registerring re-taking course, if you plan to waiver the attendence.
