二、主要学术成就<= /span>

2.1 <= /span>标志性成果(不&#= 36229;过300字)

申请人在一系列国家基金青年基金及面上项目等的资助下,长期从事心血管钙化发病机制的研究,在心血管钙化性及炎症性疾病等研究领域逐渐形成了有特色的研究方向,获得了较好的研究成果。临床结合基础研究,发现血管平滑肌细胞中代谢相关激酶AMPK在动脉粥样斑块钙化中的作用及机制;瓣膜间质细胞中内质网应激相关通路促进主动脉瓣钙化的作用及机制;血管内皮细胞中eNOS通过DHFR维持eNOS自身的生理功能的机制;巨噬细胞中iNOS调控的新机制;并对心脏炎症性疾病的发病机制进行了一定的拓展。申请人作为第一或通讯作者发表SCI收录论文15篇。

心血管钙化性疾病的发病机制复杂,多种心血管危险因素包括高脂血症、炎症刺激等,在心血管钙化的发病中起着重要作用,其可进一步作用于瓣膜间质细胞、血管平滑肌细胞、血管内皮细胞、巨噬细胞等,共同促进炎症反应的激活,导致瓣膜间质细胞以及血管平滑肌细胞向成骨细胞分化,促进心血管钙化的发生发展。申请人针对其病理生理过程中的不同组织细胞来源,进行了系列研究,发现了心血管钙化性疾病的一系列新发病机制。

 

1)阐明了血管平滑肌细胞AMPK保护动脉粥样斑块钙化形成的机制Circ Res 2016; J Diabetes Res 2019

流行病学资料显示动脉粥样斑块钙化是独立的心血管危险因素,血管平滑肌细胞向成骨细胞分化是其发病的核心环节

AMPK是生物能量代谢调节的关键分子,其表达或功能异常与糖尿病及其他许多心血管疾病的发生发展密切相关。申请人进而利用不同AMPKα亚单位敲除的小鼠,发现血管平滑肌细胞特异性AMPKα1敲除可促进小鼠动脉粥样斑块钙化,而AMPK激动剂二甲双胍则可抑制小鼠动脉粥样斑块钙化的进展。在机制上发现AMPKα1亚单位通过促进钙化相关转录因子Runx2SUMO化,进而促进Runx2降解。因而提出:AMPKα1的激活通过促进Runx2降解,抑制血管平滑肌细胞向成骨细胞分化,抑制动脉粥样斑块钙化。鉴于AMPK在糖尿病中的保护作用,申请人的该研究为糖尿病促进动脉粥样斑块钙化提出了有力的理论依据,并提出了一线降糖药物二甲双胍保护动脉粥样斑块钙化的潜在治疗可能。研究成果发表于Circ Res 2016;119(3):422并作为当期封面论文推荐

成果发表同期Circ Res由美国德克萨斯州西南医学中心Dwight Towler教授撰写的题为“AMPKα1: SUMO Wrestling Runx2 as a strategy to inhibit arteriosesclerotic calcification”编者按该研究揭示了血管钙化的代谢机制,并为高危动脉粥样硬化患者的治疗提出了新的见解。成果受邀在2016国际心脏研究学会(ISHR)中国分会暨中国病理生理学会心血管专业委员会学术大会中以大会口头报告的形式交流,引起众多国内外心血管领域学者的兴趣和关注。研究成果被包括Circ ResArteriosescler Thromb Vasc BiolKid IntJ Biol Chem等期刊引用。其中,加拿大代谢疾病研究主席(Canada Research Chair in Metabolic Diseases)与美国糖尿病协会杰出学术成果奖(American Diabetes Association Outstanding Scientific Achievement Award)获得者Gregory Steinberg教授、AMPK领域开创者Grahame Hardie教授、德克萨斯州西南医学中心Dwight Towler教授在其撰写的综述中大篇幅纳入了申请人的该研究(Trends Endocrinol Metab 2017;28(8):545, Circ Res 2017;120(11):1825Arteriosescler Thromb Vasc Biol 2017;37(2):205)。

研究成果被后续临床研究证实:申请人在临床人群中发现,与其他降糖药物相比,2型糖尿病人群使用二甲双胍与人群冠状动脉钙化积分呈负相关。研究成果发表于J Diabetes Res 2019(已接收)。法国亚眠大学Saïd Kamel教授在2型糖尿病人群中发现二甲双胍使用与下肢动脉钙化负相关,相关结果发表于Cardiovasc Diabetol 2017;16(1):24DPPDPPOS临床研究(NCT00038727)进一步显示,糖尿病前期男性使用二甲双胍可减少冠状动脉钙化积分在糖尿病前期男性,相关结果发表于Circulation 2017;136(1):52

 

2)系统性阐明了瓣膜间质细胞中内质网应激相关通路促进主动脉瓣钙化的机制Arteriosescler Thromb Vasc Biol 2013Aging Cell 2017J Thorac Cardiovasc Surg 2018Basic Res Cardiol 2012Biochim Biophys Acta Mol Basis Dis 2017

主动脉瓣钙化及其继发的钙化性主动脉瓣狭窄是心血管事件的独立危险因素,其机制不清,且目前尚无有效的药物治疗。其发病的核心过程是瓣膜间质细胞向成骨细胞分化。

内质网应激是机体受到各种应激刺激后产生的保护性反应,然而其过度激活可导致一系列信号通路,导致细胞凋亡、炎症反应等过程,参与糖尿病、动脉粥样硬化等代谢相关性疾病。但内质网应激是否参与主动脉瓣钙化的发生发展之前未有报道。

申请人发现内质网应激参与主动脉瓣钙化的发生发展。体外研究同时发现内质网应激可通过激活下游的PERK-ATF4IRE1-XBP1/JNK通路促进下游钙化相关蛋白的转录及炎症反应的激活,介导瓣膜间质细胞向成骨细胞分化,并最终导致主动脉瓣钙化的发病。研究成果发表于Arteriosescler Thromb Vasc Biol 2013;33(10):2345

申请人在此研究基础上,后续利用内质网应激下游效应蛋白CHOP敲除小鼠,验证发现CHOP敲除可抑制小鼠主动脉瓣钙化的进展,进一步阐明内质网应激激活可通过CHOP介导主动脉瓣钙化的进展。体外实验同时发现,瓣膜间质细胞CHOP的激活可促进凋亡小体释放,诱导瓣膜间质细胞向成骨样细胞分化,最终促进主动脉瓣钙化的进展。研究成果发表于Aging Cell 2017; 16(6):1334。后续申请人进一步发现组蛋白去乙酰化酶6HDAC6)在钙化的主动脉瓣中表达减少,HDAC6减少可通过激活内质网应激/ATF4通路,促进瓣膜间质细胞钙化。研究成果发表于J Thorac Cardiovasc Surg 2019; 158(2):408.

同期申请人发现糖基化终末产物(AGE)具有促进瓣膜间质细胞向成骨细胞分化的作用,该作用是通过激活糖基化终末产物受体(RAGE)所实现的。申请人进一步发现降糖药物PPARγ激动剂吡格列酮可通过下调RAGE保护家兔主动脉瓣钙化模型的进展。研究成果发表于Basic Res Cardiol 2012;107(6):306申请人后续对RAGE通路进一步研究发现,RAGE激活可激活瓣膜间质细胞内质网应激通路,促进瓣膜间质细胞向成骨细胞分化;而RAGE敲除可抑制小鼠主动脉瓣钙化的进展。研究成果发表于Biochim Biophys Acta Mol Basis Dis 2017;1863(3):781

申请人的这些研究发表后立即得到了国际同行的关注。Arteriosescler Thromb Vasc Biol论著被美国马里兰州CVpath中心Renu Vermani教授在近期ATVB关于钙化研究的亮点Arteriosescler Thromb Vasc Biol 2014;34(7):1329)中大篇幅引用;埃及曼苏尔大学Hassan I El-Sayyad教授在其编著的专著Handbook of cholesterol ISBN: 978-90-8686-276-4)第17章中也引用了申请人的该研究成果。Basic Res Cardiol论著被包括Circ ResBasic Res Cardiol等杂志引用

 

3)发现了血管内皮细胞调节自身eNOS生理功能的新机制Arteriosescler Thromb Vasc Biol 2015

内皮功能完整性在多种心血管疾病的发病中起着重要作用,而eNOS作为内皮细胞重要的功能蛋白,参与调节内皮细胞功能。有大量研究显示,eNOS功能异常是心血管钙化性疾病发病的重要促进因素

申请人发现内皮细胞eNOS产生的NO(一氧化氮),可通过S-亚硝基化(S-nitrosylation)修饰DHFR,维持内皮细胞BH4(四氢生物蝶呤)水平;BH4可进一步维持eNOS二聚体化,保持其进而生成NO的生理功能。研究成果发表于Arteriosescler Thromb Vasc Biol 2015;35(11):2366

成果发表同期Arteriosescler Thromb Vasc Biol刊发由德国美因茨古腾堡大学医学中心Thomas Münzel教授撰写的题为“Redox Regulation of Dihydrofolate Reductase, Friend or Troublemaker?”编者按该研究为氧化还原反应调节血管张力提供了全新而又重要的视角。后续香港大学Paul Vanhoutte教授、比利时鲁汶大学Jean-Luc Balligand教授在其撰写的综述中大篇幅引用该研究成果,并总结至信号通路图示中(Circ Res 2016;119(2):375Nat Rev Cardiol 2018;15(5):292)。德国法兰克福大学Ingred Fleming教授在其编著的专著Nitric Oxide(第三版)(ISBN978-0-12-804273-1)第23章中也引用了申请人的该研究成果。

 

4)阐明了巨噬细胞调节iNOS及炎症反应的新机制Circ Res 2018Mucosal Immunol 2019

巨噬细胞在多种炎症性疾病,包括心血管钙化性疾病的发病过程中起着重要作用。在主动脉瓣钙化及动脉粥样斑块钙化组织中存在大量巨噬细胞浸润,可激活一系列炎症反应,诱导瓣膜间质细胞以及血管平滑肌细胞向成骨细胞分化,促进心血管钙化性疾病的发生发展。

既往研究认为,HO-1具有抗氧化及抗炎作用。然而申请人的研究揭示,巨噬细胞来源HO-1可抑制iNOS溶酶体途径降解,进而促进炎症反应,为HO-1在炎症相关疾病中的作用提供了新的认识。研究成果发表于Circ Res 2018;122(11):1532IRF5作为I型干扰素反应的关键转录因子,在多种感染性及炎症性疾病的发病中起着重要作用。申请人发现IRF5可直接转录调节一系列M1型巨噬细胞极化相关基因Ccl4Ccl5Tnf,进而调控巨噬细胞向促炎性M1型极化,增加iNOS表达及活性,促进炎症反应。研究成果发表于Mucosal Immunol 2019;12(4):888

Circ Res的论著被意大利锡耶纳大学Lucia Morbidelli教授与美国加州大学洛杉矶分校(UCLABenjamin Bonavida教授编著的专著Therapeutic Application of Nitric Oxide in Cancer and Inflammatory DisordersISBN987-0-12-816545-4)第13章中。成果受邀在第10届同济心血管疾病高峰论坛暨华中心脏病大会基础论坛中以大会口头报告的形式交流,引起众多国内外心血管领域学者的兴趣和关注。

三、岗位工作思路&= #21450;预期目标

一、工作思路

 将在综合分析本学科发展现状和特色,以及本学科在我国同类学科中所处的学术地位、国内外同领域研究现状的基础上,根据申请人已有的工作基础和研究方向,利用青年教师学术思维活跃、精力充沛的优势,争取在临床、教学、科研等方面作出更大的贡献。

1、临床工作

 除了完成日常的临床诊疗工作外,继续以电生理介入为主攻方向,尤其是针对复杂心律失常及心衰的起搏器治疗。建立标准化房颤数据库,探讨研究持续性房颤介入治疗术式改良;针对心衰合并左束支传导组滞患者,对现有左束支起搏术士进行改良并探讨对预后的影响;以临床研究进一步推动学科在国内外的影响力。

2、教学工作

 继续将国际标准引入教学实践活动中,践行浙江大学培养 “具有优秀临床能力、创新科研思维、良好人文素养与国际竞争力”的医学卓越人才目标。继续留学生及临床医学五年制诊断学、内科学等的带教。改革现有教学模式,在以问题为中心(PBL)教学方法的基础上,进一步以系统疾病为经络,有机结合基础和临床的教学。

3、科研工作

 针对长期研究目标心血管钙化性疾病,在学科带头人领导下,探索发病机制及早期预警,并探索新的有效干预机制。在按计划完成在研的国家自然科学基金项目等科研项目同时,积极争取国家自然科学基金优秀青年项目及国家自然科学基金重点项目等,保证有足够的时间在科研第一线工作,保证按时按质完成研究计划,并进一步提升原创性论文的水平。

2.承担5年制临床医学和留学生诊断学和内科学教学;

3.积极争取包括国家自然科学基金面上项目、国家自然科学基金优秀青年基金项目等项目支持;
