二、主要学术成就<= /span>

2.1 <= /span>标志性成果(不&#= 36229;过300字)

发现临床多类重要抗菌药物耐药基因共存的“超级耐药细菌”,确认动物-环境-人类间耐药菌传播的可能;系统阐述我国基层、社区和血流感染产ESBL细菌、产NDM/KPC/IMP/MCR-1等耐药质粒流行与传播机制;深入揭示耐药菌基因组结构特征,掌握细菌耐药演变、危险因素以及耐药发生等机制;参与全国性细菌耐药监测系统建设、运行及后续应用研究,研究结果直接应用于耐药控制工作,影响政策制定,取得突出成果。任现职来承担国家级/省级课题5项,经费702万;参与国自然11项,经费663.05万,任职期间共发表SCI论文77篇(一作20篇,通讯16篇,共一6篇,共通讯3篇,参与32篇),文献引用超1350次,h-index为18(Scopus),获省科技进步二等奖和医药科技进步一等奖(均为第2完成人),任3个国家级学会青委。

一、主要学术成就

申请人主要研究方向为细菌耐药机制及基因组学研究,借助微生物学、分子生物学、基因组学、生物信息学等研究手段,重点关注人、动物、环境三大领域之间耐药菌/耐药基因的流行分布、传播规律、耐药机制及基因组特征研究。主要在以下6个方面取得一些进展:

1、率先鉴定并表征同时对多粘菌素和碳青霉烯类药物耐药的不同来源(血流感染和肠道定植)的肠杆菌科细菌。在阐明耐药菌株的耐药表型和质粒特征的同时,解析了mcr和blaNDM基因的遗传环境及传播转移机制,并对携带上述基因的耐药质粒开展了质粒可转移性和质粒完整序列研究,研究结果凸显了mcr和blaNDM未来在中国临床存在共流行的可能,也提示了肠道定植菌可能与环境来源菌株在遗传背景上存在一定的关联(相同的ST型且均携带MCR-1),且肠道定植耐药菌也有可能进一步引发临床感染。有意思的是我们还在抗生素生产厂家的污水处理厂中分离鉴定出几十株产ESBL的大肠埃希菌,且其中一株同时编码MCR-1和NDM。对比上述不同来源产MCR-1和NDM-1质粒,我们发现国内产MCR-1质粒主要为IncHI2和IncI2两种类型,而blaNDM基因主要由IncX3质粒携带。从临床感染到肠道定植再到环境中反复检出同时携带NDM和MCR-1的耐药菌,强调了对这类重要耐药菌深入监测的重要性。以上研究成果以第一作者在《Clin Infect Dis》、《Front Microbiol》、《Int J Antimicrob Agents》、《EbioMedicine》等期刊发表SCI论文5篇,被《Nature Rev Microbiol》、《Microbiome》等论文引用五十余次。

2、揭示多种革兰阴性菌对碳青霉烯类及β-内酰胺类抗生素耐药的罕见机制。首次在解鸟氨酸拉乌尔菌中发现blaKPC-2和blaIMP-4基因的共存,解析了blaKPC-2基因的上下游遗传环境以及传播转移机制,揭示了碳青霉烯耐药基因在肠杆菌科细菌中的广泛分布;我们在Klebsiella michiganensis中首次发现了blaKPC-2、blaNDM-1、blaNDM-5基因的共存,通过三代基因组测序与二代基因组测序组合研究,利用ANI分析对该耐药菌进行了精准的菌种鉴定,S1-PFGE和Southern杂交方法发现三种不同的碳青霉烯酶基因分别被126kb未知质粒、106kb IncF质粒以及46kb IncX3质粒所携带,我们的研究揭示了碳青霉烯酶编码基因在医院环境中的广泛播散,并且首次进入了K. michiganensis中,凸显了在浙江三甲医院中监测临床CRE的重要性和必要性;在泛耐药Chryseobacterium indologenes中发现并鉴定了一个新型IND 16金属β-内酰胺酶,提示了临床中不常见的C. indologenes编码了大量β-内酰胺酶。成果以一作/通讯作者在《J Antimicrob Chemother》、《Antimicrob Agents Chemother》、《Gut Pathog》等期刊发表SCI论文5篇,被《Drug Resist Updat》、《Antimicrob Agents Chemother》、《Front Microbiol》等论文广泛引用。

3. 率先开展以“One Health”策略为指导的细菌耐药研究。依托本人主持的国家重点研发计划课题以及主要参与的国自然中瑞国际合作重点项目的实施,开展了人-动物-环境界面耐药菌和耐药基因跨宿主传播研究。首次在污水厂附近的河流沉积物中发现携带blaKPC-2的柠檬酸杆菌,利用全基因组测序及质粒分析技术,我们发现河流中存在的耐药基因上下游环境与人类领域分离出的blaKPC-2存在一定相似性;此外我们还在该地点样本中分离出3株携带blaKPC-2的Aeromonas taiwanensis,通过基因组测序分析和质粒分析,我们发现尽管在同一批标本中分离检出的产KPC-2菌株,其基因结构和质粒结构完全不同,A. taiwanensis菌株携带的质粒同时编码了blaKPC-2以及qnrS2 基因,这也是首次发现同时携带这两种重要耐药基因的质粒。结合以上菌株分离的地理环境和潜在危险因素,我们猜测上述耐药质粒可能源自附近的污水处理厂。我们还对国内几十个养殖场此外,我们还在山东农村地区的农用土壤中分离出了6株携带MCR-1的大肠埃希菌,其中4株为染色体编码,暗示环境分离菌与临床分离菌在编码MCR-1的方式上存在一定差异。以上研究结果高度提示我们KPC-2 、MCR-1、ESBL等耐药基因可通过食物链或者污水处理厂排污的传播链,实现耐药菌/耐药基因从动物领域或人类领域跨界播散到环境中,如土壤或者河流中。上述相关研究成果以一作/通讯/共一/共通讯在《J Antimicrob Chemother》、《Front Microbiol》、《Sci Rep》、《Extremophiles》 等SCI 杂志发表SCI 论文11 篇。

4. 建立基于分子生物学、全基因组测序等技术的细菌基因组及耐药组分析策略。随着NGS的快速发展,高通量测序技术日益成为微生物学研究的重要推手。申请人依托实验室的NGS 测序平台致力于开展大规模的细菌基因组学研究,并且在“973 计划”子课题《横向基因转移与病原细菌致病性进化》的支撑下,系统开展了Comamonas kerstersii、Staphylococcus condimenti、Citrobacter freundii、Morganella morganii、Shigella flexneri、Enterobacter asburiae等重要细菌的基因组以及比较基因组研究,以一作/通讯/共一/共通讯在《Sci Data》、《J Antimicrob Chemother》、《Front Bioeng Biotechnol》、《Journal of Bacteriology》, 《Gut pathogens》,《Marine genomics》 等期刊发表论文16篇。

5. 积极推进重要临床病原菌的耐药机制及毒力机理研究。针对临床重要抗生素的遗传学背景开展详实研究。针对肠杆菌科细菌和鲍曼不动杆菌的替加环素耐药的机制开展基因组学及RNA-seq 研究,发现替加环素耐药与多个外派泵系统的表达水平呈正相关;对重要致病菌的毒力基因的调控开展遗传学操作、蛋白表达及动物实验,打下了扎实的耐药机制研究的技术基础。相关研究方向的成果在申请人承担的国家自然科学基金,浙江省自然基金以及以第二参与人身份参与的三个国基课题资助下,以一作/通讯/共一在《Protein & Cell》、《Gut Pathog》、《Sci Rep》等SCI 杂志发表论文6 篇。

6. 参与开全国性基层医院与血流细菌耐药监测的开展,并利用监测网开展全国性耐药菌流调研究。参与建立全国首个血流感染耐药监测(BRICS)网和血流感染细菌库,以精准分析细菌耐药监测结果,为今后细菌流行病学、耐药研究、新型抗生素的研发提供重要支持。利用上述全国性耐药监测数据和临床菌株资源,对我国血流感染中的产碳青霉烯酶肠杆菌科细菌,以及县级基层医院社区肺炎感染中的MRSA菌株开展了基因组学、耐药组学、以及临床信息的综合分析,阐明了上述两类临床重要病原菌在我国的分布和流行特征。研究结果均以Research letter形式发表在《Lancet Infect Dis》上。此外,以上相关工作还获得了浙江省科技进步二等奖和浙江省医药科技进步一等奖(均为第2完成人)。

二、贡献与创新点

细菌耐药不仅仅是临床医疗问题,它早已扩展到畜禽、水产业及环境领域中。申请人在有关项目的资助下是国内较早开展“one health” 三位一体研究耐药传播的科研人员之一,通过系统的对临床病人、动物以及环境来源的耐药菌开展了相关的耐药机制和跨领域传播研究,对阐明CRE、产MCER-1肠杆菌科细菌以及MRSA等其他重要病原菌和耐药菌在我国多领域的流行分布和传播机制做出一定的贡献。申请人深度参与了2 个中国-瑞典国际合作项目,在项目中本人均负责中瑞双方的联络沟通、课题组织和研究计划的实施,系统的参与了双方的协调沟通,为中瑞在耐药领域合作的开展起到了桥梁的作用,推动合作双方在前期基础上成功申请到两个国家自然科学基金的项目,通过就合作我们也加深了对瑞典耐药控制运行模式的理解和借鉴,建立了国际交流平台,为中国细菌耐药研究的国际化交流合作提供了一定的助力和借鉴经验。此外,申请人在国内3个国家级协会中担任青年委员,在3个一级/核心期刊中担任编委/通讯编委/青年编委,在国内细菌耐药研究领域发挥积极作用。

三、科学价值或社会经济意义

细菌耐药是全球性的严重公共卫生问题,2016年国家卫健委等13个部委联合颁布了“耐药控制国家行动计划”,在全国开展医疗机构耐药控制的管理活动。尤其是2016年我国主办的杭州G20峰会中,把耐药控制作为主要内容之一,纳入领导人公报中,成为全球政治议题。以上政治活动说明细菌耐药研究是关乎人口健康和经济发展的全球性问题,具有深远的科学研究价值和重大的社会经济意义。本人入职以来系统参与建立了国家细菌耐药监测信息体系,开展了全国大型医疗机构细菌耐药监测、基层医疗机构细菌耐药监测以及血流感染细菌耐药监测,进行了深入的分子流行病学和细菌耐药机制等全面研究,成果间接在我国耐药控制政策制定、临床耐药控制以及医院抗菌药物合理使用等众多领域发挥了作用,为全国公共卫生做出了积极贡献,是对习总书记“健康中国”战略最好的实践。


三、岗位工作思路及预&= #26399;目标

1、科研方面:在后期研究中注重发表高质量高影响力的工作,改变科研发展模式,专注于集中所有的精力和研究资源深刻阐明耐药形成、发展和播散的机制问题,争取短期内发表影响因子超过10或者超过20的细菌耐药及基因组研究领域的高质量研究论文,尽量减少Stamp  collection类似的工作,产出更多标志性成果,进而出版专著、教材,力争获得高层次的成果奖励。
2、学科建设交流方面:积极为浙江大学“双一流”建设工作贡献自己的力量,积极参与国内外相关的学术交流建设活动,尤其是在国际学术活动场合中积极抢占中国学术发言高地,推动中国耐药研究领域在国际中的发声。在基于中瑞国际合作的基础上,持续有效地拓宽与美国、英国等不同合作单位的深入合作,协助重点实验室领导做好各项学科建设工作,在学术交流中发挥推动作用。
3、积极承担行政管理和社会服务中的义务工作:踊跃参与医学院、学科及国重室的其他公益服务工作等。
