二、主要学术成就<= /span>

本人从事临床病原微生物的耐药机制和致病机理研究,致力于探索抗生素耐药细菌的耐药机制和病原细菌致病机理,曾前往澳大利亚Monash大学生物医学中心完成一年的访问学者科研工作。自任现职以来,共主持国家自然科学基金3项,包括2项面上项目和1项青年项目,主持浙江省自然科学基金1项、浙江省医药卫生科技项目1项。以第一作者发表SCI收录文章3篇,影响因子分别为6.425、4.615、4.715,SCI他引次数最高达到单篇72次。获得浙江省科学技术进步奖一等奖1项(第二完成人)、浙江省医药卫生科技奖一等奖1项(第二完成人)。现任浙江省医学会医学微生物与免疫学分会青委会副主任委员,国家自然科学基金一审评委,以及《Int J Antimicrob Agents》等SCI期刊的同行评议工作。

本人在浙江大学获得生物信息学专业博士学位,现在浙江大学医学院附属邵逸夫医院从事临床病原微生物的耐药机制和致病机理研究,依托于浙江省临床微生物学重点学科、浙江省微生物技术和生物信息研究重点实验室,近年来在重要临床病原细菌感染,特别是多重耐药肺炎克雷伯菌、鲍曼不动杆菌引起的感染相关研究领域取得了丰硕的研究成果。自任现职以来,以第一作者发表SCI收录文章3篇,另外作为主要参与人发表SCI文章33篇,所发表文章大多刊登于本领域国际知名学术期刊上,包括《Clin Microbiol Infect》、《Int J Antimicrob Agents》、《Antimicrob Agents Chemother》、《Lancet Infect Dis》等等,其中2篇文章分别在2011年和2017年获得中国百篇最具影响国际学术论文奖。共主持国家自然科学基金3项,包括2项面上项目和1项青年项目,另外主持浙江省自然科学基金1项、浙江省医药卫生科技项目1项。获得浙江省科学技术进步奖一等奖1项(第二完成人)、浙江省医药卫生科技奖一等奖1项(第二完成人)。现任浙江省医学会医学微生物与免疫学分会青委会副主任委员,并连续七年担任国家自然科学基金一审评委,以及《Int J Antimicrob Agents》等SCI期刊的同行评议工作。另外本人作为课题组的学术骨干,积极和学科带头人(PI)进行协作,参与培养博士研究生9名,硕士研究生4名。

临床病原细菌耐药性危机是全球共同关注的重要人类健康问题,如果不能找到治愈耐药性“超级细菌“的方法,人类可能会重新回到无药可用的黑暗时代。本人长期致力于多重耐药菌感染领域研究,从临床感染病例中发现新的耐药问题,开展细菌耐药机制、多重耐药菌流行特征及致病机理的系列研究,充分发挥基础与临床结合的优势,为多重耐药菌的流行控制和感染诊治提供了科学指导。

创新点(一):长期研究临床重要多重耐药菌和新出现耐药菌的耐药机制,揭示我国重要耐药菌系列新机制。碳青霉烯抗生素是革兰阴性菌重症感染的最后防线,课题组首次在亚太地区报道了产KPC-2型碳青霉烯酶的耐药肺炎克雷伯菌感染病例,在此基础上,本人所做的研究,首次阐明了我国KPC-2基因传播的新机制(通过Tn3-Tn4401复合转座子传播),在国内最早解析了携带KPC-2基因的质粒全序列,明确了我国KPC酶的遗传结构与国外具有显著差异,率先揭示了我国这种重要碳青霉烯酶耐药基因的独特传播结构。此项研究通过大量耐药细菌的表型、基因型分析,以及耐药质粒全序列分析,发现耐药细菌除通过医院感染途径造成传播外,更主要是通过耐药基因的水平转移传播耐药性,该研究成果从分子水平深入阐明了我国KPC型“超级细菌”的传播机制。代表性论文: 蒋琰(第一)、俞云松(通讯)等,Antimicrob Agents Chemother. 2010; 54(9): 3967-3969.  影响因子4.715,SCI他引72次。

创新点(二):长期研究多重耐药的“超级细菌”的感染诊治策略,关注碳青霉烯耐药革兰阴性菌感染的治疗药物的共同耐药问题。磷霉素作为一种广谱杀菌剂近年来被应用于碳青霉烯耐药细菌的感染治疗,但随着临床使用的增加,发现磷霉素对碳青霉烯耐药菌株的耐药率也在不断上升。本人的研究发现同一质粒同时携带磷霉素修饰酶及碳青霉烯酶是肠杆菌科细菌对碳青霉烯类抗生素和磷霉素共同耐药的主要原因,质粒介导的基因水平转移在耐药性方面发挥了重要作用。本人的研究表明,质粒编码的fosA3基因对磷霉素常常表现为高度耐药性,并且,fosA3基因往往通过IS26-fosA3-IS26转座元件和Tn3-Tn4401介导的KPC-2型碳青霉烯酶基因共同水平传播,加快了耐药性在院内传播的速度。此项研究将对防止磷霉素耐药的扩散及寻找合适的碳青霉烯耐药细菌联合治疗药物尤为关键。代表性论文:蒋琰(第一)、俞云松(通讯)等,Int J Antimicrob Agents 2015; 45(1): 66-70.  影响因子4.615,SCI他引39次。

创新点(三):明确了我国临床重要多重耐药菌的分子流行病学特征,国内首次应用最新的高通量基因组测序技术追踪和溯源重要“超级细菌“院内流行和传播的规律,首次建立了细菌基因组分型、溯源方法及数据分析的一整套流程。本人的研究首次报道了我国院内碳青霉烯耐药肺炎克雷伯菌最主要的流行克隆,此克隆在很多省市地区暴发流行,是我国医疗机构需要重点防范和控制的耐药细菌克隆。通过结合高通量基因组测序技术,本研究进行了耐药菌流行克隆的追踪和溯源,提升了耐药菌溯源技术水平,揭示了产KPC-2菌株流行的本质。与现有分型技术相比,基于细菌基因组的分型和溯源方法分辨率更高,能更准确区分同一克隆的菌株,便于菌株克隆传播的实时追踪与快速溯源,同时可以构建公开访问的在线网络数据库以追踪重要病原菌的流行状况,从而为细菌感染性疾病的播散提供预警。代表性论文:蒋琰(第一)、俞云松(通讯)等,Clin Microbiol Infect. 2015; 21(11): 1001-7.  影响因子6.425,SCI他引16次。

创新点(四):在国际上首次应用混合质粒组结合最新一代高通量基因组测序技术,追踪质粒介导水平转移元件与耐药基因之间的相互联系。基因水平转移介导耐药性的快速传播是耐药细菌研究领域所关注的热点问题,高通量基因组测序是追踪耐药基因水平转移的最好手段,但通常所用的第二代基因组测序技术测序片段短,不利于分析较大的水平转移元件,而新一代测序技术价格昂贵,分析大量样本成本太高。本研究首次在国际上采用混合质粒组测序的方法,通过提取多样性不一的质粒混合,构建有代表性的质粒组文库,再结合长片段的第三代基因组测序技术,一次性获得大量的质粒组片段序列。这样即为耐药基因水平转移分析提供了大量足够长度的原始数据,又解决了成本太高的问题,为以后同类研究提供了良好的思路。大量长片段序列的生物信息学分析也是本研究的一大挑战,本人通过前期积累的良好的生物信息学技术基础,逐一解决了技术难题,分析结果充分展示了肺炎克雷伯菌编码质粒的水平转移元件的多样性,也明确了这些元件转移耐药基因的不同方式和联系。代表性论文:蒋琰(第一)、俞云松(通讯)等, Genomics Proteomics Bioinformatics,2019 Aug,已接收,影响因子6.597。

除了已经取得的创新成果,本人还在临床重要病原细菌的致病机制研究领域做了大量的工作,近几年先后申请了国家自然科学基金面上项目《鲍曼不动杆菌种优势性:不动杆菌属不同菌种生存、毒力与耐药性比较研究》和《鲍曼不动杆菌血流感染的致病机制》,以及浙江省自然科学基金《致化脓性肝脓肿肺炎克雷伯菌毒力因子研究》,旨在围绕肺炎克雷伯菌、鲍曼不动杆菌等病原细菌的致病性开展研究,通过和澳大利亚Monash大学的Anton Peleg教授合作,并依托浙江大学分子生物学平台、生物信息技术、动物感染平台、激光共聚焦显微镜平台以及各种新技术,已经开展了大规模、高水准的分子机制研究工作。


三、岗位工作思路及预&= #26399;目标

本人所在岗位旨在围绕临床出现的科研问题,进行系统性基础研究,并基于对分子机制的理解,指导应用型转化和临床诊治策略的选择。本岗位工作思路应先注重基础性研究,从临床实际工作中遇到的问题出发提出科学假设,利用分子生物学技术、新的组学技术、动物研究技术等多方面研究平台,多层次多角度地进行科学系统论证。在本人具体工作领域上,要注意全面地围绕耐药细菌感染、细菌如何致病等关键临床问题开展工作,旨在解决和回答临床多重耐药菌感染所带来的治疗难题。其次工作思路中还应该加强同各个层次科技人员的合作,包括和本课题组PI的紧密合作,和同领域各学科带头人的合作,和本领域活跃在一线的科研人员的合作,另外还包括协助PI培养博士、硕士研究生。最后工作思路中还应注意从科学基础研究回到临床,切实解决临床问题,和附属医院感染科、呼吸科、重症医学科等相关科室展开紧密联系,理论研究成果能确实回答临床上亟需解决的问题,注重科研开发转化,用科技产品为临床服务。

今后的预期目标包括:

(1)继续围绕临床重要多重耐药细菌进行耐药机制研究,探索决定新出现耐药性的关键耐药基因,阐明耐药基因的水平传播机制和耐药细菌克隆的院内播散机制,找寻抑制细菌耐药性的关键性药物。

(2)深入研究临床重要病原菌的致病机制,探索决定病原菌对人类致病的关键毒力因子,理解细菌致病性的作用机理、基因通路和表达影响因素,找到防御细菌致病因子的临床策略。

(3)科学理论研究成果能切实回答临床上亟需解决的问题,完成部分科研成果的开发转化。

(4)在国际上有影响力的学科杂志上发表研究文章,同时申请国家级重要研究课题,并注重对博士、硕士研究生的培养。

本人在临床病原细菌耐药机制和致病机理方面所做的研究,具有很好的社会效益和经济效益。首先对于多重耐药的“超级细菌”分子流行病学和耐药基因传播机制的研究,为我国临床工作者面对这些耐药细菌感染时提供了药物选择和治疗方案的理论基础,这些研究直接领导了我国四项耐药菌感染诊治专家共识的建立。其次本人关于耐药机制、耐药流行特征的研究成果,已被临床和制药企业广泛应用于病人诊治和新药研发,关于KPC型碳青霉烯酶的研究,促成了浙江惠迪森制药有限公司开发了三种新碳青霉烯酶抑制剂,广泛应用于“超级细菌”感染的治疗,成果的推广应用减少了耐药菌感染的发生,提高了耐药菌感染患者的治愈率。
