二、主要学术成就<= /span>

2.1 <= /span>标志性成果(不&#= 36229;过300字)

申请人通过建立孕妇队列探索孕期与产后母婴健康问题,主要标志性成果包括:

  1. 首次报道维生素DVitD)通路基因与季节对VitD缺乏存在交互作用;孕早中期VitD缺乏明显增加空腹血糖异常型妊娠糖尿病风险,首次发现孕前超重或肥胖的孕妇出现VitD缺乏发生妊娠糖尿病的风险更高;明确孕妇补充常规推荐剂量400IU VitD不足于改变VitD缺乏的现状;提出了孕妇VitD补充的推荐剂量;这些研究成果为合理补充VitD提供了有价值的循证依据。

  2. 发现孕早期单位体重的增加对出生体重的影响最大;首次提出了孕期亚甲减的诊断标准随孕期而不同,而一般人群的血脂异常的诊断标准不适用于孕妇人群;这些研究成果为科学管理孕妇健康提供了第一手的证据。

本人的主要研究方向为慢性病分子流行病学研究,包括代谢性疾病和孕期并发症、胎儿与婴幼儿健康病因学的分子流行病学研究,在该研究领域产生了以下三个方面的主要贡献和相关的创新之处:

1. 首次报道维生素DVitD)通路基因与季节对VitD缺乏存在交互作用;孕早中期VitD缺乏明显增加空腹血糖异常型妊娠糖尿病风险,首次发现孕前超重或肥胖的孕妇出现VitD缺乏发生妊娠糖尿病的风险更高;明确孕妇补充常规推荐剂量400IU VitD不足于改变VitD缺乏的现状;提出了孕妇VitD补充的推荐剂量;这些研究成果为孕妇合理补充VitD提供了有价值的循证依据。

 迄今,对于孕妇是否需要补充VitD仍存在争议,且对于VitD补充的剂量也尚无定论。围绕这些临床问题,我们展开了相对系统的一系列研究。我们发表在2019年《Clinical Nutrition》的研究报道了舟山孕妇队列人群中,VitD缺乏或不足是很普遍的现象,尤其是孕早期VitD缺乏或不足率高达90%,中、晚期分别为76.3%71.0%;研究发现,不管孕期是否补充VitD,血25(OH)D水平都随着孕周的进展而逐步升高,且补充400 IU VitD >3/周才能有效地增加血25(OH)D水平,但补充400 IU VitD ≦3/周的孕妇在孕晚期仍有61.7%孕妇存在VitD缺乏或不足,这也间接地支持了我们发表在2019年《European Journal of Clinical Nutrition》研究首次推荐孕妇每天补充2250 IU VitD才能达到充足的VitD水平的结论。针对VitD营养状况的影响因素方面,我们探索了环境因素、生活行为习惯和遗传因素等对孕妇VitD水平的影响,在2019年《Clinical Nutrition》的研究还发现,首胎、冬春季、孕前超重或肥胖、孕期增重多或脑力劳动等因素增加了VitD缺乏的风险;另一项发表在2018年《Clinical Nutrition》的研究中,首次在孕妇人群中报道了VitD合成、转运和代谢通路基因多态性(GC, CYP3A4, CYP24A1, NADSYN1/DHCR7),尤其是GC基因的多态性,对孕期VitD缺乏存在明显的影响,并且这些基因与环境因素对孕妇的维生素水平存在显著交互效应,如:CYP3A4 CYP24A1基因与季节对VitD缺乏存在交互作用,即携带这些基因突变的孕妇在冬春季节VitD缺乏更为严重,提示这类孕妇在冬春季节补充大剂量的VitD预防VitD缺乏更为必要。另外,在2019年《Clinical Nutrition》刚接受的一篇文章中,发现孕早中期VitD缺乏只增加空腹血糖异常型妊娠糖尿病风险,对OGTT 1h/2h血糖升高型妊娠糖尿病的发病风险没有影响,且首次发现这种效应在孕前超重或肥胖的孕妇中更加明显(OR=3D9.6195%CI3.30-28.0。妊娠期糖尿病是影响母婴健康的重要影响因素,2018年发表在《Journal of Diabetes and Its Complications》的研究显示,妊娠糖尿病显著增加了巨大儿的发生率;这些研究成果为孕妇合理补充VitD提供了有价值的循证依据。

2. 明确了儿童和成人中VitD不足或缺乏非常的普遍,并采用现场试验确认每天补充400IU VitD对改善儿童和成人VitD营养状态效果不佳,同时提出了不同人群VitD补充的推荐剂量。

 因为人群VitD的营养状态受到饮食、年龄、肥胖和日光暴露等因素影响,各个地区人群VitD的营养状态差异很大,且儿童和成人每天补充常规推荐的400IU VitD能否有效改善VitD营养不良的现状仍无定论;据此本课题组开展了相关的研究,发表在2017年《Nutrient》的研究结果显示,浙江省儿童VitD的营养状态随年龄变化很大,新生儿VitD水平受到孕妇低VitD水平的影响,导致80%以上的新生儿都存在VitD不足或缺乏;不过,出生后VitD水平就迅速升高,到5个月龄的时候达到高峰,持续到1岁左右,这期间85%儿童VitD都达到充足水平;然而,这之后就开始下降,到6-7岁时候与新生儿的水平相当,之后一直持续在这样的低水平状态;可见大部分儿童都存在VitD不足的问题。影响儿童VitD不足的因素分析中,我们发现冬春季和疾病都明显增加儿童VitD低下的风险。而对于成人的VitD的营养状态,我们也进行了调查,在另一篇发表在2017年《Nutrient》的研究中,发现办公室白领人群中的VitD营养状态也不容乐观,其中84%的人VitD营养不良。针对儿童和成人VitD营养不良如此普遍的现状,我们采用现场试验设计,每天给予小学生和白领办公室人群补充400IU VitD,但是研究结果并不理想,补充后虽然儿童和成人25(OH)D的水平稍有提高,但仍有70%以上的人25(OH)D处于不足或缺乏水平,说明在儿童和办公室人群中每天补充常规剂量400IU VitD也是不够的,该结果与我们2019年在《European Journal of Clinical Nutrition》报道的结果相吻合,该研究结果显示为了达到充足的VitD水平,推荐儿童每天补充1340 IU VitD,亚洲成年人补充2026IU VitD/。这些研究结果为儿童和成人补充VitD提供了参考依据。

 

3. 发现孕早期单位体重的增加对出生体重的影响最大;首次提出了孕期亚甲减的诊断标准随孕期而不同,而一般人群的血脂异常的诊断标准不适用于孕妇人群;这些研究成果为科学管理孕妇健康提供了第一手的证据。

近些年来,我国胎儿出生体重异常情况仍高居不下,2018年我们分析了2002-2015年舟山市新生儿出生体重数据,结果发现该人群胎儿出生体重有下降的趋势,但低出生体重儿和巨大儿发生率分别为2.34%和9.42%。而孕期体重和孕期增重是胎儿出生体重最重要的影响因素,我们2017年发表在《预防医学》研究也支持该结论。对于该地区孕妇孕期体重增加的情况,发表在2017年《疾病监测》的研究对此进行了探讨,从2000-2015年间,舟山地区的孕妇总体孕期增重变化不大,在该孕妇人群中44.4%的孕妇体重增重过多,这个结果可以部分解释巨大儿的发生率高达9.42%的现象;进而在2019年《Archives of Gynecology and Obstetrics》的研究中,我们首次报道了孕妇孕早期每单位体重的增加对胎儿出生体重的贡献最大,其次是孕中期,孕晚期的贡献最小;这些研究成果提示要改变居高不下的胎儿出生体重异常率,孕期体重管理不仅要控制孕期体重和孕期总增重,还要结合孕期进行统一管理。

另外,我们还关注孕妇的甲状腺素水平和血脂水平,发表在2017年《Gynecol Endocrinol》的研究数据显示,TSH水平随着孕期的进展而逐步升高,而FT3FT4水平随着孕期而下降,按照目前的亚甲减的诊断标准,孕妇亚甲减发病率就随着孕期的进展而增高,并且在不同年龄的孕妇中差异很明显;这将会导致在孕中晚期提高了亚甲减的误诊率,首次提出孕妇亚甲减的诊断标准不能采用统一固定的激素水平切点,而应该根据孕期的不同而变化。孕妇血脂水平异常尚缺乏统一的诊断标准,本课题组研究发现,与非孕期同年龄、同BMI的女性相比,孕妇的血脂水平明显增高,按照一般成年女性的血脂异常的诊断标准评价孕妇的血脂水平,90%孕妇都要被诊断为高血脂,因此我们利用14000例孕妇血脂的数据,建立了孕妇血脂的推荐诊断标准。同时,我们发表在2017年《Taiwanese Journal of Obstetrics & Gynecology》的Meta分析显示,血脂水平与早产的发生风险呈正相关;这些结果提示制定孕期合理的血脂异常诊断标准用于指导孕期健康管理的重要性。

 

三、岗位工作思路&= #21450;预期目标

围绕浙大“双一流”建设的总目标,形成岗位工作思路和设定预期目标:

1. 科研工作

(1)进一步加强已有的舟山和义乌两个孕妇队列人群的建设,建立长效的随访机制,构建完善的人群研究平台;

(2)继续深入研究孕期与产后母婴健康问题,加强维生素D对孕期并发症、出生结局和儿童生长发育等的作用研究,尤其是维生素D对妊娠糖尿病的机制研究,希望在该领域有突破;

(3)5年内争取发表IF>10的高水平学术论文1-2篇。

  1. 教学工作

(1)《流行病学》、《医学科研方法》、《循证医学》都是应用型学科知识,对提高医学生的临床能力和科研能力都至关重要,但是医学生只对理论知识有一定的了解,缺乏应用这部分知识的能力;因此如何提高医学生对这些科目知识的应用能力是一个重要的问题,因此计划申报该领域的教改课题,提高教学质量;

(2)投入更多的精力到教学活动中,积极引进高效教学方法,提高教学质量。

(3)提高研究生培养的能力,激发研究生的学习和科研潜能。

3. 学科建设和社会服务

(1)积极参与学科建设,帮助提高学科的知名度和影响力。

(2)利用专业特长,为医疗领域的师生提供更为专业的培训和咨询服务。
