二、主要学术成就<= /span>

2.1 <= /span>标志性成果(不&#= 36229;过300字)

 带领西京小肠移植团队,瞄准换肠(小肠移植)”保肠(自体小肠移植)”延肠(STEP)”等世界难题,在十分简陋的条件下,完成各类复杂小肠移植49例,其中活体小肠移植30例,1年生存率90%5年达80%;脑死亡供者小肠移植4例(其中2例为腹部六脏器联合移植);15例自体小肠移植,该项技术是我们的原创性技术。西京团队的努力彻底改变了我国小肠移植长期停顿的状态。当前,活体小肠移植和自体小肠移植保持国内外领先地位。

一、小肠移植临床和实验研究

小肠是含有丰富淋巴组织的空腔有菌器官,长期以来移植成功率低,远期疗效远劣于其它实质脏器,是器官移植领域急需攻克的世界性难题。1999年,我们小肠移植团队克服重重困难,在国内毫无先例可寻的条件下,成功施行了国内第一例亲缘性活体小肠移植手术,目前病人已经健康生存20年,成为国际上存活时间最长、生活质量最好的"换肠人"。1995年出国后早期师从诺贝尔奖评选委员会委员、国际著名器官移植权威Carl G. Groth教授,学习临床器官移植技术(肝、肾和胰腺)并攻读博士学位。毕业后赴美继续深造,先后在著名移植权Thomas E.StarzlAndreas TzakisGeorge Burke IIITomaoki KatoKareem Abu-Elmaged等带领下,主要从事小肠、肝小肠和腹部多器官簇移植临床工作。美国匹兹堡大学医疗中心(UPMC)一度是国际上最大的器官移植中心,迄今共完成各类小肠移植手术超过600例。我在担任主治医师期间,主刀完成单独小肠移植手术25例,肝小肠联合移植4例,多器官簇移植3例。

2011年回国后,相继成功实施了国内首例非亲缘性小肠移植(2011)、亚洲首例同卵双生小肠移植(2013)、世界首例血型不合小肠移植(2013)和机器人小肠移植(2014)等多项移植手术,填补多项国内外空白,国家唯一的小肠移植培训基地落户西京医院

研究内容和主要学术贡献:

1.HLA抗体在小肠移植排斥反应中的作用及其机制研究:2010年我以第一作者在国际上首先报道了HLA抗体,特别是供者特异性抗体(Donor-specific antibody, DSA)与排斥反应之间的密切关联,提出有效抑制B细胞及其分泌的抗体才能保证移植小肠的长期存活(Am J Transplant 2010; 10(Suppl4):162Am J Transplant 2012;12(11):3047-3060)。随后,国际上其它中心进一步证实了我们的研究发现,阐明HLA抗体的作用机制、阻止抗体产生成为当前小肠移植领域的研究热点。

2.小肠和多器官簇移植术后移植物抗宿主病(GVHD)的防治:GVHD是小肠移植术后严重的、有时致命性的并发症。我们的一项大宗病例回顾性研究表明,移植术后发病率大约9-10%,免疫系统尚不成熟的婴幼儿、多器官簇联合移植和切除受者脾脏是发生GVHD的主要危险因素(Transplantation 2011;91(2):219-24)。一旦明确诊断,死亡率接近80%。因此,我们提出预防为主同时维持供受者之间免疫平衡的个体化治疗策略。回国后完成49例小肠移植手术(30例活体、4例脑死亡供者、15例自体),均未发生GVHD

3.移植肝脏在降低小肠移植术后排斥反应中的作用及其机制研究;随着我们对HLA抗体的新认识,抗体介导的排斥反应(Antibody-mediated rejection, ABMR)日益受到重视。我们的研究表明,不含肝脏的单纯小肠移植,术后急慢性排斥反应是影响疗效的关键因素,而含肝脏的小肠移植或多器官簇移植,术后感染是造成预后不良的主要因素。因此,我们提出含肝脏和不含肝脏的小肠移植是两类性质迥异的移植类型,前者的治疗策略重点在于防治感染,需要适度降低免疫抑制剂用量,而后者应该是增强免疫抑制。在严重致敏病人(高水平HLA抗体),可以考虑肝小肠联合移植(Transplantation 2014;996):1265-1272Gastroenterol Rep (Oxf) 2018;6(1):29-37

4.小肠移植免疫耐受的实验和临床研究:诱导受者对供者小肠形成特异性免疫耐受是解决小肠移植诸多问题的根本措施。研究发现,通过建立供受者异基因混合嵌合体能够诱导皮肤移植耐受。小肠是一个高免疫原性器官,诱导小肠移植耐受的途径和形成机制尚不明确。在国科金的资助下,我们利用动物模型,通过:1)骨髓细胞移植建立供受者异基因嵌合体,优化移植耐受诱导方案,明确嵌合体水平与移植耐受的关系; 2) 以异基因嵌合体为基础,建立小鼠小肠移植耐受模型,探讨小肠移植耐受的形成途径和其产生的中枢和外周免疫调节机; 3)通过供受者嵌合体诱导灵长类动物小肠移植耐受,阐明其形成机制和技术研发的安全性。在临床上,我们通过术后多次输注供者外周造血干细胞调节供受者间免疫平衡,初步结果表明,供者造血干细胞可能通过提高移植肠粘膜局部的免疫调节细胞(Foxp3+ Tregs),从而降低排斥反应,提高治疗效果。目前,相关研究工作和论文撰写正在进行中。

5.小肠移植手术前后肠道菌群的变化与排斥反应的关系研究:小肠是有菌的高免疫原性器官,降低其高免疫原性是解决排斥反应的重要途径。供体和受体之间组织相容性抗原差异是引起排斥反应的基本因素,而对肠道微生态变化在移植排斥反应中的作用尚未阐明。肠道微生物是造成小肠高免疫原性的又一个重要因素,肠道微生物可能通过活化抗原呈递细胞诱导效应T细胞增殖分化,促进小肠排斥反应,导致移植失败。在国科金的资助下,我们利用小鼠小肠移植模型,通过:1)高通量测序技术分析肠道菌群及其变化对排斥反应的影响和规律,探讨菌群负荷和种类改变降低排斥反应的方法;2)研究肠粘膜浸润淋巴细胞和引流淋巴结抗原呈递细胞及其信号传导,阐明肠菌促进排斥反应的分子生物学机制和关键环节;3)探讨菌群干预、菌群移植并联合免疫抑制剂阻止排斥反应的方法,为进一步提高小肠移植的治疗水平提供理论依据和新的途径,此项研究工作目前正在进行中。

二、自体小肠移植

起源于胰腺、腹膜后或肠系膜根部的良恶性肿瘤如果累及肠系膜大血管,传统手术方法无法切除或达到根治性切除。近年来,基于小肠移植技术,我们采取肿瘤连同胰腺、十二指肠和小肠整体“搬家”的办法,在体外进行灌洗之后切除肿瘤,再将健康的部分小肠重新移植到体内的自体小肠移植技术。迄今共施行此类重大疑难手术15例,手术成功14例,最长的一例胰头恶性肿瘤病人,随访8年无肿瘤复发迹象。目前,我们报道的病例数量最多,手术成功率和存活率方面居国际领先水平,成为世界上少数几家掌握该项技术的单位之一(J Gastrointest Surg 2019 Apr 1. doi: 10.1007/s11605-019-04204-1)自体小肠移植的“保肠”技术,既实现了彻底清除肿瘤,最大限度保住自体小肠的目的,又避开了小肠移植后抗排斥的难题,为腹部外科探索出了一种全新术式。我们拟进一步扩大病例数量和病种,规范手术适应症,完善手术操作要领,为这项技术向全国推广提供支持和保障

三、小肠整形延长术(Serial transverse enteroplasty, STEP)

一般而言,小肠100厘米以下会出现严重的消化吸收问题,不足40厘米需要终生输液或进行小肠移植。能否使长度在40-100厘米之间的小肠变长,避免终生输液或小肠移植,是移植团队攻关的又一方向。我们在国内率先开展了国际上先进的“小肠整形延长术(Serial transverse enteroplasty, STEP)”。这项“延肠”的技术,不仅避免了“换肠”和“保肠”等小肠移植的技术难题,同时极大降低了这类手术的风险,改善了病人的生存质量。

一位26岁的小伙由于“短肠”,体重只有50斤,被称作“骷髅人”,我们采取在小肠上做多个纵形切口,使其像弹簧一样横向延长的办法,成功把小肠从70厘米延长到120厘米。一年后患者体重上升到120多斤,饮食起居正常。“延肠”技术,不仅避免了“换肠”和“保肠”技术的科学难题,而且极大降低手术风险,有效提升了患者生存质量。目前共完成此类手术4例,远期疗效良好。我们拟进一步扩大手术适应症,规范手术操作要领,探讨减少术后并发症的措施,为这项技术向全国推广提供支持和保障

三、岗位工作思路&= #21450;预期目标

“终末期肠病”又称不可逆性肠功能衰竭,是由各种原因引起的小肠严重功能障碍或小肠切除术后导致的吸收面积不足,最常见的原因是肠系膜血管血栓形成、肠扭转坏死、炎症性肠病和儿童先天性肠道畸形。近年,该病增速迅猛、病情凶险、病死率高,严重危害人类健康。在深入研究“终末期肠病”发病机制和病理特征基础上,进一步发展完善小肠移植、自体小肠移植、小肠整形延长(STEP)、肠道微生态移植和干细胞移植等治疗技术,提高我国“终末期肠病”的诊治水平,延长生存期,改善生活质量,从根本上降低该病死亡率。在聘任期内,建成我国“终末期肠病”基础研究的基地、临床诊治的中心、人才培养的摇篮及国际交流的平台。我们的主攻方向是:

一、小肠移植、肝小肠联合移植和腹腔多器官簇移植

小肠移植是解决“终末期肠病”的最终有效手段。小肠移植术后常致严重排斥反应,生存率远低于其它脏器,是移植领域的世界性难题。本中心1999年实施国内首例亲属间活体小肠移植,目前患者已健康生活20年,移植肠功能良好,是世界上存活时间最长的“换肠人”。本中心已完成各类疑难小肠移植术27例,疗效跻身国际领先水平,已有16篇论文在国际杂志发表,也是目前国内开展小肠移植最多的单位。本项目将:1.加强小肠移植的基础理论研究;2.完善小肠移植术后排斥反应的早期诊断和治疗标准;3.探索降低小肠移植术后排斥反应的新方法、新技术;4.扩大小肠移植病种和数量,五年内在规模和疗效上成为世界上活体小肠移植最大中心;5.依托肝胰移植中心,开展肝小肠联合移植和腹腔多器官簇联合移植;6.面向全国培养人才和推广技术。

三、小肠移植术后肠道微生态研究

肠道微生态是近年来世界上发展迅猛的研究前沿,大量证据表明其与肠功能维护高度相关。西京消化病医院在国内率先开展数百例重症炎性肠病等的肠道微生态移植治疗,取得显著疗效,已有11篇相关论文在国际杂志发表。本中心应用小檗碱抑制肠液积聚及电解质分泌、控制炎症反应、保持肠道微生态的平衡,受到国际关注。《小檗碱预防炎症性肠病严重并发症的期临床研究》已获美国国家癌症研究所基金,这是该所资助我国的第一个研究项目。加盟浙大一院之后,我们将继续与西京消化病医院密切合作:1.加强肠道微生态的基础和临床研究,阐明小肠移植术后肠道微生态的变化和排斥反应炎症反应的关系和机制;2.树立整体的治疗观念,积极寻找有效的中医中药并探索其配方及治疗机制,促进移植术后供受者之间的免疫平衡;3.扩大病种和数量,提高疗效,形成标准,并向国内外推广。
