二、主要学术成就<= /span>

长期致力于肝移植术后新发糖尿病的发病机制和防治研究,取得原创性成果包括:1.创新性地从供肝角度开展研究,阐明供肝TCF7L2基因突变介导Wnt和PI3K/AKT等多信号通路引起肝移植术后新发糖尿病发生的分子机制;2.发现免疫抑制剂Tacrolimus调控的移植肝MicroRNA代谢特征谱,揭示移植肝糖代谢稳态失衡的新机制。以第一或通讯作者在Am J Transplant、J Hepatol等发表SCI论文19篇,受邀在移植领域Top期刊Transplantation发表综述;获得浙江省自然杰出青年基金,并主持3项国家自然科学基金等课题;获得国际肝移植协会“Rising Star”荣誉(全球6位);参与起草《中国肝移植受者代谢病管理专家共识》和《中国肝移植受者肾损伤管理专家共识》,研究成果被纳入多项国内外临床诊治指南共识并实现转化应用;作为主要完成人之一获得浙江省科学技术一等奖。

申请人在长期肝移植临床和基础研究中,较早地瞄准肝移植术后新发糖尿病(new-onset diabetes after liver transplantation, NODALT)这一严重影响移植疗效的难点问题,突破以往局限于受者参数和胰腺功能的研究,从“供肝质量”、“受者状态”、“免疫抑制副作用”这影响肝移植疗效的三大关键特征入手,开展发病机制及转化研究在3项国家自然科学基金和浙江省自然杰出青年基金等的资助下,共发表SCI论文50余篇,均在国内完成,其中以第一或通讯作者在Am J TransplantJ Hepatol等发表SCI论文19篇,总IF>90;在移植领域Top期刊Transplantation发表综述;连续6届国际肝移植协会(ILTS)大会做报告,并获得“Rising Star”和“Young Investigator”荣誉;重视科学问题的临床回归,研究成果被纳入4项国内外临床诊治指南共识并实现转化应用;作为主要完成人获得浙江省科学技术一等奖(4/13)。具体学术成绩、创新点及其科学意义阐述如下:

(1) 创新性地从供肝角度开展研究,阐明供肝TCF7L2基因突变引起肝移植术后新发糖尿病发生的分子机制J Hepatol 2013Transplantation 2016 Liver int 2016Int J Cancer 2016等)

以往关于NODALT发生机制的研究多集中于移植受者因素,申请人基于前期基础提出“移植肝作为受者的新代谢中心在维持血糖稳态中发挥重要作用”这一科学假说,在连续两项国家自然科学基金(8110032181470892)的资助下,创新性地从供肝基因背景入手开展NODALT的发病机制研究。

申请人研究发现供肝TCF7L2(糖尿病关键风险基因)的rs290487位点T→C突变,导致NODALT的发病风险增加了2.8倍;疾病预测模型分析中,供肝基因型的加入使得临床模型的预警能力显著提高。研究结果对于供肝分配和术后治疗方案精准选择具有重要指导价值:对于高风险病人,应避免移植携带TCF7L2基因变异的供肝;对于植入了rs290487位点突变供肝的受者,术后应尽量选择无激素、低剂量的免疫抑制方案。新方案预期将NODALT的发病率从25%下降到<5%,从而提高移植受者生存时间和生活质量。研究成果发表于肝病领域Top期刊J Hepatol2013,第一作者,IF:12.5)。

申请人进一步从分子和亚分子水平及基因间相互作用研究TCF7L2基因突变引起NODALT发生的分子机制,发现rs290487位点T→C突变可引起TCF7L2基因剪切变异和蛋白表达下降,胰岛素信号通路IRS2/AKT/GSK活性下降,葡萄糖转运蛋白GLUT2和糖异生关键调节子PEPCK表达下降,显著增加胰岛素抵抗,并诱发肝源性糖尿病;还发现肝脏TCF7L2基因3’端rs290481rs290487rs290489三位点 G-C-A突变促进肝癌发生,而供肝基因突变可改变移植肝微环境促进肝移植术后肝癌复发。上述研究阐明了汉族人群rs290487位点突变导致TCF7L2基因剪切变异、调控WntPI3K/AKT等多信号通路引起肝糖代谢稳态失衡的内在机制,并提出了TCF7L2/Wnt信号通路介导的移植肝糖代谢稳态失衡的促癌作用。研究成果发表于Gene2013,第一作者)、Int J Cancer2016,第一作者,IF:6.5)和Sci Rep2016a,通讯作者;2016b,通讯作者)。

申请人关于“供肝基因背景与NODALT”的新发现受到国内外同行重视,2016年在移植领域Top期刊Transplantation发表综述The Origin of New-Onset Diabetes After Liver Transplantation: Liver, Islets, or Gut?Transplantation 2016,第一作者,指出供肝、受者胰腺和肠道微生态调控移植受者血糖稳态,引起NODALT发生。国际知名肝病专家Eshraghian教授在J Hepatol撰写评论指出“这是一项设计严谨、结论可信的研究,……,使我们更深入的了解肝移植术后糖尿病的发病机制”。申请人进一步受邀在J Hepatol2013,第一作者,IF:12.5)撰文阐述研究构思和创新发现。

 

(2) 发现免疫抑制剂Tacrolimus调控的移植肝MicroRNA代谢特征谱,揭示移植肝糖代谢的稳态失衡新机制Am J Transplant 2017, Medicine 2016等)

免疫抑制剂TacrolimusTAC)为肝移植受者主要免疫抑制维持药物(占95%以上),而高浓度的TAC被认为是诱发NODALT的主要因素。以往研究大都关注于TAC对胰岛的直接毒性,而申请人发现TAC能引起明显胰岛素抵抗效应(Gene2013,第一作者),并基于这一发现创新性地从移植肝糖脂代谢角度研究TAC引起胰岛素抵抗、诱发NODALT的分子机制。

申请人研究发现TAC能显著改变肝细胞MicroRNA表达谱,通过上调let-7miR-183,降低靶通路IRS2/PI3K/AKT活性,导致肝胰岛素抵抗,并降低TCF7L2FOXO1表达减少糖异生,从而造成空腹血糖不稳;TAC还能上调miR-26a/b,提高PPARGLXRASREBP1蛋白表达,引起游离脂肪酸合成增加和肝细胞甘油三酯沉积。进一步研究发现供肝PPARA基因敲减可靶向上调TAC代谢酶CYP3A4,加速肝脏TAC代谢并降低血药浓度,对移植肝糖脂代谢稳态起到保护性作用。上述研究从MicroRNA-mRNA-蛋白水平揭示了TAC调控的移植肝糖脂代谢稳态失衡新机制,并为分子治疗提供新靶点,有望减少免疫抑制剂毒副作用降低NODALT发生率。研究成果发表于移植领域Top期刊Am J Transplant2017,第一作者,IF:6.1)和Medicine2015,第一作者)等。

申请人获得第22ILTS大会Rising Star荣誉(全球共6位),关于TAC对移植肝糖脂代谢的研究多次在国际大会上进行报告交流。ILTS前任主席、国际著名肝移植专家Jan Lerut教授评价:“申请人是一位富有创新精神的年轻外科医师和科研工作者……创新性地从供肝角度研究NODALT……发现了TAC诱发移植受者糖代谢紊乱的新机制”。Am J Transplant审稿人评论:“这项有趣的研究评估了供肝miRNANODALT发生中的重要作用……为阐明NODALT发生的内在机制提供了新颖思路”。

 

(3) 发现中国受者人群肝移植术后新发糖尿病等的重要临床病理特征,提出防治新方案并被采纳应用Liver Int 2016等)

申请人在大量临床一线工作中,发现“中国肝移植人群NODALT增加胆道狭窄和肝癌复发风险”这一重要表现特征,提出了“严控围手术期血糖水平、免疫抑制剂最小化、缩短ICU时间、密切检测CMV感染”的NODALT临床防治新方案。研究成果发表于Liver Int2016,第一作者)。

申请人进一步对NODALT相关并发症如肝移植术后肾功能损伤等进行基础和临床研究,提出基于Cystatin C的肾功能损伤诊断新模型和基于MELD评分的移植前降期新方案。研究成果发表于Sci Rep2017,第一作者)、PloS One2012,第一作者)、World J Gastroenterol2012,第一作者)和Dig Dis Sci2011,第一作者)等。

申请人重视科学问题的临床回归,研究成果已开展转化应用并被纳入《中国肝移植受者代谢病管理专家共识(2015)》、《中国肝移植受者肾损伤管理专家共识(2017)》《Acute-on-chronic liver failure: consensus recommendations of the APASL 2014(亚太肝病组织肝衰竭诊治指南2014版)》等项临床诊治指南和专家共识,作为主要完成人获得浙江省科学技术一等奖4/13)。Liver Int期刊前主编、国际著名肝病学专家Hongqun Liu教授评论:“申请人的研究新发现……有望降低NODALT发生率并最终提高肝移植受者生存率和生活质量”。
